Designing and sending forms

Medical history, consent and questionnaires: designing them, sending them, follow-ups.

Three kinds of form

Settings → Forms holds every form patients fill in. Your medical history form is asked for before a patient's first visit and again every year. A consent form is signed before each appointment for the services that need it: choose it for a service in Settings → Services & prices, under Consent form. A questionnaire is sent when you choose: from a patient's record, or automatically after a visit. You can have one medical history form; as many consent forms and questionnaires as you like.

Designing a form

Press Start designing, starting from a template or a copy of one of your forms. Give it a name, choose when it's used, then add sections and questions: short or long answers, choose one, choose any, yes or no, dates, numbers, phone numbers, "tick to agree" statements and information text for patients to read. Tick Required for questions that must be answered, and use the arrows to move questions and sections. Tick Ask for a signature for a form patients sign, such as a consent form.

Show a question only sometimes

Show only if shows a question only when an earlier yes or no, or choice, question has a certain answer: for example, "Where does it hurt?" only when "Any pain?" is yes. Hidden questions are never required, and nothing typed in them is kept.

Preview, drafts and versions

Preview shows the form as patients will see it. Save draft keeps your changes without patients seeing them; Publish makes them live as a new version. Answers already given keep the version the patient saw, so the record always shows the exact questions they answered. A form you no longer use can be archived; its answers stay on patients' records.

Sending a form

On a patient's record, under Forms, choose a published form and press Send. It goes by email, and by text if they've agreed to texts; for a child it goes to their parent. The link shown can also be handed over or opened on a tablet in the clinic. The link works for 30 days. Until it's filled in, the form is listed as "Waiting for", where you can cancel it.

Follow-ups after a visit

Under Follow-ups after a visit, choose a questionnaire, how many days after the visit, and which services (none ticked: any). When a visit is marked completed, the questionnaire is sent that many days later, once per visit. Changing a visit or cancelling it changes or stops the follow-up.

Answers

Answers are encrypted and saved on the patient's record, with any signature. Clinicians can open them (reception can see that a form is done, not what it says), and every view is in the access log. The owner can download every answer to a form as a spreadsheet from the form's page; that download is logged too.
