Foot charts
Marking callus, corns, verrucae, ulcers, nail conditions and deformities on a drawing of both feet, with what changed since last time.
Adding a foot chart
Open a clinical note you're writing and press Foot chart under Foot chart and assessments. You'll see both feet from above (top of the feet) and from below (the soles), left foot on the left, big toes towards the middle and toes numbered 1 to 5.
Tap where the finding is: a toe, a toenail, the ball of the foot, the arch, the heel, the edges or the ankle. Choose What it is (callus, corn, verruca, fissure, ulcer, a nail condition such as involuted, ingrowing or fungal nail, a deformity such as hallux valgus or hammer toe, and more) and press Add to the chart. A marker with a short code (Ca for callus, V for verruca and so on) appears where you tapped. Nails offer nail conditions first. If your tap landed in the wrong place, change Where in the list; "Between the toes", the Achilles and the lower leg are only in that list.
Every change is saved with the note straight away. Tap a marker, or the pencil next to it in the list below, to change or remove it.
Notes and photos
Each finding can have notes (size, grade, what you did) and a photo: one of the patient's clinical photos, with photos of the same site listed first (★). Add photos under Photos on the note, or from Files and photos.
Next time
On the next note, Start from the last chart copies the last chart's findings, so you only update what changed. The patient's record shows the latest chart under Foot health; See changes over time lists what's new, still there and gone since the chart before, with every earlier chart.
Signing and printing
The chart is locked when you sign the note; to correct it, amend the note (the amendment starts with a copy). Foot health has Print or save as PDF for a clean copy of the charts, assessments and wound trends.
Who sees it
Like clinical notes: clinicians and the owner, never reception. Everything is encrypted with your clinic's own key and every view is recorded in the audit log. It's in the patient's subject access export and the clinic's data download.
